1. (1) A non-resident of this state who is licensed or certified in another state and who wishes to apply for a six-month temporary permit to perform one or more specific appraisal assignments in Utah shall:

   1. (a) evidence that each specific appraisal assignment is covered by a contract to provide appraisals;

   2. (b) submit an application as provided by the division and including the following:

      1. (i) name of the client;
      2. (ii) specific property addresses to be appraised;
      3. (iii) types of property being appraised; and
      4. (iv) estimated time to complete each assignment;
   3. (c) complete and submit a qualifying questionnaire as provided by the division;
   4. (d) sign an irrevocable consent to service authorizing the division to receive service of any lawful process on behalf of the applicant in any non-criminal proceeding arising out of the applicant's practice as an appraiser in this state;
   5. (e) pay a nonrefundable application fee in the amount established by the division; and
   6. (f) provide the starting date of the appraisal assignment for which the temporary permit is being sought.

2. (2)(a) A non-resident is limited to two temporary permits per calendar year, each of which may be extended one time for an additional six-month period if the assignments for which the permit is issued have not been completed within the original six-month term of the temporary permit.

   1. (b) A temporary permit may be extended by submitting the forms required by the division.
