1. (1) A registered health care services platform shall notify the Division in writing, in a form approved by the Division, within ten business days after any of the following changes to the health care services platform's registration information:

   1. (a) website domain name;
   2. (b) contact person for the registration; or
   3. (c) legal business name, including any change in "doing business as" (DBA) name or entity type.
2. (2) A health care services platform is not required to notify the Division or submit a new application for registration because of a change in ownership of the health care services platform.
