1. (1)(a) current Utah licensure as a dental hygienist; or

   1. (b) documentation of meeting Utah requirements for licensure as a dental hygienist;

2. (2) completion of a program of training in the administration of local anesthesia, including nitrous oxide, that:

   1. (a)(i) is accredited by the CODA; or

      1. (ii) is the substantial equivalent of Subsection (2)(a)(i) provided in a continuing education format by an ADA accredited school; and
   2. (b) documentation of successful completion of the program by a letter from the program director, or equivalent; and

3. (3)(a) a passing score on the WREB, CDCA, or SRTA written anesthesia examination; or

   1. (b) a current, active in good standing license to administer local anesthesia in another state in the United States; and
4. (4) current CPR or BCLS-BLS certification.
