1. (1)(a) Under Subsection 58-47b-306(1)(b), a licensee shall require that each client complete a client intake form before the first session; and

   1. (b) before each new session, a licensee shall review the most recently completed intake form with the client and confirm all information is current.

2. (2) Under Subsection 58-47b-306(1)(b), a licensee's intake form shall include at least the following:

   1. (a) the name of the massage establishment or the sole practitioner;
   2. (b) the date of the session;

   3. (c) the client's:

      1. (i) first and last names;
      2. (ii) birth year;
      3. (iii) contact information; and
      4. (iv) emergency contact;
   4. (d) if the client is seeking a massage for a specific health issue;

   5. (e) the client's health history including:

      1. (i) each major medical issue;
      2. (ii) each allergy;
      3. (iii) a current acute injury as defined in Subsection R156-47b-102(2);
      4. (iv) a current subacute injury as defined in Subsection R156-47b-102(21); and
      5. (v) if the client is currently under the treatment of a physician;

   6. (f) the client's right to:

      1. (i) disrobe in privacy and to the client's comfort level;
      2. (ii) ask questions;
      3. (iii) identify areas of the body the client would like the licensee to avoid;
      4. (iv) modify the treatment;
      5. (v) stop treatment; and
      6. (vi) be properly draped;

   7. (g) policies regarding:

      1. (i) cancellation; and
      2. (ii) no show; and
   8. (h) signed consent to treatment.
