1. (1)(a) the original affidavit of compliance; and

   1. (b) a list of known subcontractors who provided service, labor, or materials under the general contractor.
2. (2) When an affidavit of compliance is disputed, the owner must submit evidence demonstrating compliance with the requirements specified in Subsection 38-11-110(2)(c)(ii).

The following supporting documents shall, at a minimum, accompany each homeowner application for a certificate of compliance seeking protection under Subsection 38-11-110(1)(a)(ii):
