1. (1) Under Subsection 58-31b-303(1)(b) and Section R156-31b-302b, an applicant for LPN licensure who graduated from a nonapproved nursing education program shall demonstrate that the nursing education program completed by the applicant is equivalent by submitting:

   1. (a) a CGFNS Credentials Evaluation Service Professional Report that is acceptable to the Division and the Board; or
   2. (b) documentation of meeting the endorsement requirements of Section 58-1-302.

2. (2) Under Subsection 58-31b-303(2)(b) and Section R156-31b-302d, an applicant for RN licensure who graduated from a nonapproved nursing education program shall submit:

   1. (a) a CGFNS Certification Program Verification Letter; or
   2. (b) documentation of meeting the endorsement requirements of Section 58-1-302.
