1. (1) The prescription drug shall be dispensed:

   1. (a) during regular inpatient hospital pharmacy hours, by a pharmacist; or
   2. (b) outside of regular inpatient hospital pharmacy hours, by the prescribing practitioner using an appropriately labeled pre-packaged drug.

2. (2) Labeling for a prescription under Section 58-17b-610.6 shall at a minimum include:

   1. (a) prescribing practitioner's name, facility name, and telephone number;
   2. (b) patient's name;
   3. (c) name and strength of medication;
   4. (d) date given;
   5. (e) instructions for use; and
   6. (f) beyond use date.
3. (3) Applicable data of controlled substances dispensed shall be reported to the Utah Controlled Substance Database.

In accordance with Section 58-17b-610.6, the guidelines for a hospital pharmacy to dispense to an individual who is no longer a patient, on the day discharged from the hospital setting, are established in this section.
