1. (1) The department shall assign a docket number to each notice of agency action and, if appropriate, to each request for agency action.

2. (2) At a minimum the docket number shall consist of:

   1. (a) a letter code identifying where the matter originated, as follows:

      1. (i) CORP-Corporations;
      2. (ii) CP-Consumer Protection;
      3. (iii) DOPL-Professional Licensing, including additional designations that the division may implement for the Utah Professionals Health Program, lien recovery fund, or other programs;
      4. (iv) NAFA-New Automobile Franchise Act;
      5. (v) PVFA-Powersport Vehicle Franchise Act;
      6. (vi) RE-Real Estate;
      7. (vii) AP-Real Estate Appraisers;
      8. (viii) MG-Mortgage; and
      9. (ix) SD-Securities;
   2. (b) a numerical code indicating the calendar year the matter arises; and
   3. (c) another number indicating chronological position among notices of agency action or requests for agency action filed during the year.

3. (3) The department shall give each adjudicative proceeding a title in substantially the following form:
