1. (a) A copy of each document filed in a contested case shall be served upon all parties or their authorized representatives by sending a copy properly addressed to each party by:

   1. (1) first-class mail; or
   2. (2) email.

2. (b) A copy of each document must also be filed with the department by:

   1. (1) email;
   2. (2) fax; or
   3. (3) first-class mail.
3. (c) A certificate of service shall accompany each document.
