A proposal must be sealed and contain:
- (1) the contract number;
(2) an organizational chart containing names of the:
- (A) prime provider and any subproviders proposed for the team and their contract responsibilities; and
- (B) key personnel proposed for the contract and percent of their time to be dedicated to the project.
- (3) a summary of the project team qualifications;
- (4) a clear and concise statement demonstrating an understanding of the scope and requirements of the project;
- (5) a description of how the provider proposes to meet the project's goals and objectives;
- (6) a proposed schedule of work which describes the provider's internal methods for schedule control;
- (7) an audited financial statement dated no later than the preceding fiscal year;
- (8) supporting information, such as charts, photographs, graphs, resumes, and references; and
- (9) other pertinent information addressed in the notice.
