A proposal must be sealed and contain:

1. (1) the contract number;

2. (2) an organizational chart containing names of the:

   1. (A) prime provider and any subproviders proposed for the team and their contract responsibilities; and
   2. (B) key personnel proposed for the contract and percent of their time to be dedicated to the project.
3. (3) a summary of the project team qualifications;
4. (4) a clear and concise statement demonstrating an understanding of the scope and requirements of the project;
5. (5) a description of how the provider proposes to meet the project's goals and objectives;
6. (6) a proposed schedule of work which describes the provider's internal methods for schedule control;
7. (7) an audited financial statement dated no later than the preceding fiscal year;
8. (8) supporting information, such as charts, photographs, graphs, resumes, and references; and
9. (9) other pertinent information addressed in the notice.
