1. (a) The Commission shall mail to each party notice of:

   1. (1) the Commission's decision;
   2. (2) the violation;
   3. (3) the amount of any penalty assessed;
   4. (4) if applicable, the amount of any investigative costs; and
   5. (5) the parties' right to file a motion for rehearing.
2. (b) The notice shall be mailed to the party's last known address, as shown by the Agency's records.
3. (c) The Commission shall enter a written penalty order for the payment of any penalty or investigative costs the Commission has assessed.
