Each shipment of plants and plant products moved within the state shall have legibly marked upon it in a conspicuous manner and place all of the following:

1. (1) the name and address of the shipper;
2. (2) the name of the person to whom the shipment is forwarded or shipped or the name of his agent;
3. (3) the name of the country, state, or territory where the contents were grown; and
4. (4) a statement of its contents.
