The facility shall obtain and record the following information at the time the resident is admitted into the facility:

1. (1) date and time of admission;
2. (2) name;
3. (3) nicknames and aliases;
4. (4) social security number;
5. (5) last known address;
6. (6) document(s) authorizing legal admission;
7. (7) admitting offense;
8. (8) name of attorney;
9. (9) name, title, and signature of delivering individual;
10. (10) sex;
11. (11) race;
12. (12) date of birth;
13. (13) citizenship;
14. (14) place of birth;
15. (15) current education level;
16. (16) last school attended;
17. (17) name, relationship, address, and phone number of parents, legal guardians, or custodians; and
18. (18) primary language of resident and resident's parent, legal guardian, or custodian.
