The facility shall have an emergency preparedness plan that includes, but is not limited to, severe weather, natural disasters, disturbances or riots, national security issues, and medical emergencies. The plan shall address:

1. (1) the identification of key personnel and their specific responsibilities during an emergency or disaster situation;
2. (2) agreements with other agencies or departments; and
3. (3) transportation to pre-determined evacuation sites.
