1. (a) Each facility that is authorized to use chemical restraints must have a use of force review board consisting of the facility administrator and other staff, as designated by the juvenile board in policy.
2. (b) The use of force review board reviews each use of force involving chemical restraints no later than 14 calendar days after the restraint.

3. (c) The use of force review board reviews all available documents, videos, and sources of information to:

   1. (1) determine whether facility policies were properly applied;
   2. (2) determine whether documentation was accurate and complete;
   3. (3) identify training needs; and
   4. (4) identify ways to expand prevention efforts.
4. (d) Written document of the names of all in attendance, a list of each incident reviewed, and any corrective actions recommended must be created and saved for each meeting.
