Any person or entity seeking to contract with the board on a competitive basis or otherwise shall disclose all previous donations occurring within the preceding two years to the board or any other state agency. The disclosure shall include the following information:

1. (1) the nature and value of the donation; and
2. (2) the date the donation was made and the recipient. If the donation is ongoing, the last date that the donation was available to the agency shall be used to determine the date of the donation.
