1. (a) Each applicant must provide its full name, post-office address, telephone number, and social security or federal identification number.
2. (b) If the applicant is a partnership, it must be designated by the firm name followed by the words "a partnership."
3. (c) If the applicant is acting as trustee for another, it must be designated by the trustee's name followed by the word "trustee."
4. (d) If one other than the named applicant executes the application, the name, position, post-office address, and telephone number of the person executing the application must be given.
