All correspondence with the State Office of Risk Management concerning a claim must contain the following information:

1. (1) the name of the injured employee;
2. (2) the social security number of the injured employee;
3. (3) the date of injury;
4. (4) the name of the employing agency;
5. (5) the director's claim number and the commission number, if known.
