1. (a) A provider must notify HHSC in writing at least five days before permanent closure of the operation.

2. (b) The provider must include in the written notice:

   1. (1) the date of permanent closure;
   2. (2) the reason for closing;
   3. (3) the location of individual records, both active and inactive; and
   4. (4) the name and address of the individual records custodian.
