What must the provider agency's written plan for staff development include?

The provider agency's written plan for staff development must include the:
- (1) schedule for training, including the length of time;
- (2) curriculum, including specific topics;
- (3) training objectives;
- (4) method of training; and
- (5) names of the instructors.
