A CDSA must:

1. (1) compile a report in accordance with the format provided by DADS addressing each service delivered through the CDS option, including the actual number of hours or units of service delivered;

2. (2) provide the report no less than quarterly, and monthly if requested, to:

   1. (A) the employer or DR; and
   2. (B) the case manager or service coordinator; and
3. (3) provide a copy of the report to DADS, upon request by a DADS representative.
