1. (a) In accordance with Chapter 156 of the Act, a physician license must be registered with the board and renewed every two years after it is issued.
2. (b) A renewal notice will be sent to the physician's address of record at least 60 days prior to the expiration date of the registration.

3. (c) The physician must:

   1. (1) complete the renewal form;

   2. (2) pay the renewal fee and any additional fees, as applicable:

      1. (A) initial biennial permit - $456.00;
      2. (B) subsequent biennial permit - $452.00;
   3. (3) verify and update their physician profile; and
   4. (4) provide any other relevant information requested.
