1. (a) To be eligible for a certificate of compliance, a distributor, jobber, supplier, or wholesaler must:

   1. (1) submit complete business, facility, and contact information in a manner prescribed by the department; and
   2. (2) pay the fee required under §97.71.
2. (b) A change in the name or contact information of the owner or operator must be submitted to the department within 30 calendar days.
3. (c) A certificate of compliance is valid for two years from the date of issuance by the department.
