1. (a) The department will issue an applicant, whose application has been approved, a registration containing the registered provider's name, registration number, and expiration date.
2. (b) A continuing education provider registration issued by the department remains the property of the department.
3. (c) The department will issue a duplicate registration upon written request by the provider, in a form and manner prescribed by department, and upon payment of the duplicate/replacement fee under §112.110.
