1. (a) A dental support organization must submit a statement of correction if, during the quarter, any information provided in the registration changes.

2. (b) A statement of correction must include the following information:

   1. (1) The legal name of the dental support organization;
   2. (2) The date of the last filed registration;
   3. (3) Any identification number assigned by the secretary of state assigned to the dental support organization; and
   4. (4) A statement identifying the information that has changed.

3. (c) A dental support organization that is required to submit a statement of correction in accordance with subsection (a) shall do so according to the following schedule:

   1. (1) First quarter (January 1 - March 31) - Statement of correction due not later than the 45th day after March 31.
   2. (2) Second quarter (April 1 - June 30) - Statement of correction due not later than the 45th day after June 30.
   3. (3) Third quarter (July 1 - September 30) - Statement of correction due not later than the 45th day after September 30; and
   4. (4) Fourth quarter (October 1 - December 31) - Statement of correction due not later than the 45th day after December 31.
4. (d) Each statement of correction shall be signed by a person authorized to act by or on behalf of the dental support organization.
