Sec. 162.123. INFORMATION REQUIRED ON BLENDER'S RETURN

The monthly return and supplements of each blender shall contain for the period covered by the return:

1. (1) the number of net gallons of gasoline received by the blender during the month, sorted by product code, seller, point of origin, carrier, and receipt date;
2. (2) the number of net gallons of product blended with gasoline during the month, sorted by product code, type of blending agent if no product code exists, seller, and carrier;
3. (3) the number of net gallons of blended gasoline sold during the month and the license number or name and address of the entity receiving the blended gasoline; and
4. (4) any other information required by the comptroller.
