An application for certification as an inspector shall:

1. (1) be made on a form prescribed and provided by the department; and

2. (2) state:

   1. (A) the name of the applicant;
   2. (B) the address of the applicant's residence and place of employment;
   3. (C) the applicant's driver's license number; and
   4. (D) other information required by the department.
