1. (a) A person must submit an application to the board to obtain a permit under this chapter.

2. (b) The application must state:

   1. (1) the name and address of the person requesting the permit;
   2. (2) the location and wellhead elevation of the well or proposed well;
   3. (3) the amount of water being withdrawn or proposed to be withdrawn; and
   4. (4) any other information necessary for the board to control and prevent subsidence in the district.
3. (c) The board shall set a reasonable fee for processing an application. The application must be accompanied by the fee.
