The executive director shall give notice of the commission's order to the person. The notice must:

1. (1) include the findings of fact and conclusions of law, separately stated;
2. (2) state the amount of any penalty imposed;
3. (3) inform the person of the person's right to judicial review of the order; and
4. (4) include other information required by law.
