An applicant for a certificate must submit to the advisory board written evidence, verified by oath, that the applicant has completed:

1. (1) an approved four-year high school course of study or the equivalent as determined by the appropriate educational agency; and
2. (2) a respiratory care education program approved by the advisory board.
