A person may apply for a license under this chapter by submitting an application to the executive council. The application must:

1. (1) be on a form prescribed by the executive council; and
2. (2) contain statements made under oath regarding the applicant's education and experience and any other information required by the executive council that qualifies the applicant for a license.
