1. (a) A license holder shall file with the board:

   1. (1) the license holder's mailing address;
   2. (2) the address of the license holder's residence;
   3. (3) the mailing address of each office of the license holder; and
   4. (4) the address for the location of each office of the license holder that has an address different from the office's mailing address.
2. (b) Not later than the 10th day after the date of a change in the information required to be filed with the board under Subsection (a), the license holder shall file with the board a written notice of the change.
