An applicant for a license must submit to the department:

1. (1) an application that includes:

   1. (A) the applicant's name;
   2. (B) the applicant's business address;
   3. (C) the applicant's permanent mailing address; and
   4. (D) any other relevant information required by the department; and
2. (2) an application fee.
