1. (a) A company license holder shall notify the department not later than the 14th day after the date of:

   1. (1) a change of address for the company license holder's principal place of business;
   2. (2) a change of a name under which the company license holder does business; or
   3. (3) a change in the company license holder's officers or partners.

2. (b) A company license holder shall notify the department in writing not later than the 14th day after the date a branch office:

   1. (1) is established;
   2. (2) is closed; or
   3. (3) changes address or location.
