A system shall ensure that each employee and retired employee participating under this chapter is issued a certificate of coverage that states:

1. (1) the benefits to which the participant is entitled;
2. (2) to whom the benefits are payable;
3. (3) to whom a claim must be submitted; and
4. (4) the provisions of the plan document, in summary form, that principally affect the participant.
