Denial by a small or large employer health benefit plan issuer of an application from a small or large employer for coverage from the issuer or cancellation of or refusal to renew coverage by a small or large employer health benefit plan issuer must:

1. (1) be in writing; and
2. (2) state the reason or reasons for the denial, cancellation, or refusal to renew.
