1. (a) Any person may apply to the commissioner for and obtain a certificate of authority to organize and operate a health maintenance organization.

2. (b) An application for a certificate of authority must:

   1. (1) be on a form prescribed by rules adopted by the commissioner; and
   2. (2) be verified by the applicant or an officer or other authorized representative of the applicant.
