1. (a) An applicant for a license under this chapter must submit to the department an application on a form prescribed and provided by the department, and include as part of or in connection with the application any information that the department reasonably requires, including information about the applicant's:

   1. (1) identity;
   2. (2) personal history;
   3. (3) experience; and
   4. (4) business record.
2. (b) The application must be accompanied by the fee required by Section 4101.057.
