This chapter applies only to a county that:

1. (1) is not served by a hospital district; and

2. (2) is:

   1. (A) a county that:

      1. (i) has a population of more than 46,000 and less than 50,000; and
      2. (ii) is adjacent to the county containing the state capital;

   2. (B) a county that:

      1. (i) has a population of 900,000 or more; and
      2. (ii) borders two counties, each of which has a population of two million or more; or

   3. (C) a county that:

      1. (i) has a population of one million or more;
      2. (ii) contains all or part of a municipality with a population of one million or more; and
      3. (iii) is adjacent to a county with a population of 2.5 million or more.
