A license issued under this chapter shall be displayed in a conspicuous place in the designated place of business and must show:

1. (1) the name and address of the licensee;
2. (2) the name of the owner or owners, if different from the information provided under Subdivision (1);
3. (3) the license expiration date; and
4. (4) the types of services authorized to be provided under the license.
