To the extent practicable using existing revenue, the executive commissioner, by rule, shall develop and the commission shall implement a plan to:

1. (1) identify recipients of financial assistance that are at risk of exhausting their benefits under Section 31.0065; and
2. (2) provide referrals for the recipient and the recipient's family to appropriate preventive and support services, including faith-based services.
