1. (a) A consumer who elects to purchase optional insurance must sign a statement that:

   1. (1) indicates the consumer's election; and
   2. (2) describes the term, premium, and type of insurance purchased.
2. (b) The statement may be a part of the credit document or a part of a separate document.
