1. (a) In addition to the information required by Section 252.002, a campaign treasurer appointment by a candidate must include:

   1. (1) the candidate's telephone number; and
   2. (2) a statement, signed by the candidate, that the candidate is aware of the nepotism law, Chapter 573, Government Code.
2. (b) A campaign treasurer appointment that is filed in a manner other than by use of an officially prescribed form is not invalid because it fails to comply with Subsection (a)(2).
