In addition to the information required by Section 3.005, the certificate of formation of a professional entity must state:

1. (1) the type of professional service to be provided by the professional entity as the purpose of the entity; and

2. (2) that the professional entity is a:

   1. (A) professional association;
   2. (B) professional corporation; or
   3. (C) professional limited liability company.
