A disclosure statement must contain:

1. (1) a detailed description of the actual services the seller undertakes to perform for the purchaser; and

2. (2) if the seller promises to perform services in connection with the placement of products, equipment, or supplies at a location:

   1. (A) the full nature of those services; and

   2. (B) the nature of any agreements to be made with the owners or managers of that location.
