1. (1) Change of Name - An individual registered with the Board must notify the Board in writing within thirty (30) days of a name change. The notice must provide both the old and new names and must reference the individual’s profession, board, and certificate number.
2. (2) Change of Address - A licensee or certificate holder must notify the Board of a change of address within thirty (30) days of such change. The notification must be in writing and include both the old and new addresses along with the licensee’s name, profession, and license or certificate number.
