1. (1) The fees are as follows:

   1. (a) Premises Application Fee - A non-refundable fee to be paid by the owner of the facility seeking a premises permit. It must be paid each time an application for a premises permit is filed.
   2. (b) Initial Inspection/Biennial Inspection Fee - A non-refundable fee to be paid at time of application for premises permit and every two years at time or renewal.
   3. (c) Premises Permit Fee - A non-refundable fee to be paid at time of application by a facility owned by a non-veterinarian.
   4. (d) State Regulatory Fee - To be paid by all facilities owners at the time of application and renewal application.
   5. (e) Premises Permit Renewal Fee - A non-refundable fee to be paid by all applicants for renewal of a Premises Permit.
   6. (f) Late Renewal Fee - A non-refundable fee to be paid when a facility owner fails to timely renew a premises permit.
   7. (g) Reinspection Fee (follow-up) - A non-refundable fee to be paid when a facility does not pass the initial inspection or the biennial inspection.
2. (2) All fees shall be established by the Board. Fees may be reviewed and changed at the discretion of the Board.

3. (3) Fee Schedule: Amount

   1. (a) Premises Application $ 25.00
   2. (b) Initial Inspection/Biennial Inspection$200.00
   3. (c) Premises Permit $360.00
   4. (d) State Regulatory Fee $ 10.00 (biennial)
   5. (e) Premises Permit Renewal $160.00 (biennial)
   6. (f) Late Renewal $ 80.00
   7. (g) Reinspection Fee(follow-up) $150.00
4. (4) All fees may be paid in person, by mail or electronically by cash, check, money order, or by credit and/or debit cards accepted by the Division of Health Related Boards. If the fees are paid by certified, personal or corporate check they must be drawn against an account in a United States Bank, and made payable to the Tennessee Board of Veterinary Medical Examiners.
