5. (5) Each party requesting a Business Entity database must swear under the penalty of perjury that it will not use, or knowingly allow another to use, any purchased database to market goods or services in a manner that implies that the person using or employing the purchased database is part of, or associated with, a unit of any governmental entity.
6. (6) The Division of Business and Charitable Organizations may prescribe, and shall furnish on request, a form that a party requesting a database must use for any purchase under this rule.
