(1) Upon proper application by a voter for an absentee by mail ballot, the registrar shall process the application in accordance with T.C.A. §2—6—202, and shall issue to the voter the following supplies:
- (a) An appropriately pre-punched absentee ballot.
- (b) A write-in envelope.
- (c) A styrofoam backing for use when punching a card.
- (d) A ballot guide for absentee voting, indicating the punch portions for each candidate for each office in the selection.
- (e) A set of instructions for absentee voting by mail, which adequately describes the procedures to be followed to cast a punch card ballot by mail.
- (f) A disposable ballot punch.
- (g) Return envelope.
- (h) Affidavit.
