In order to qualify for receipt of a Pearl Harbor Survivor special plate, the applicant shall present either a Pearl Harbor Survivor Association, Inc. membership card or a copy of a Department of Defense form which shows that the applicant is considered to be a Pearl Harbor survivor by the Department of Defense.
